This policy explains what happens to the information you send us through this website.
What this policy covers
This policy covers this website only: the contact and consultation forms, the email addresses and phone numbers published here, and the technical records our host keeps when a page is served.
It does not cover your clinical record. Health information created once you are a client is protected health information, handled under HIPAA and described in the practice's Notice of Privacy Practices, which you receive at intake.
What we collect
- What you type into a form: your name, email address, phone number, and whatever you choose to tell us about what you are looking for.
- What you send us directly by email or phone, including voicemail.
- Standard server records: IP address, browser and device type, the pages requested and the time of the request.
- Analytics about how pages are used, in aggregate, so we can see which pages people find and which they do not.
What we do with it
We use what you send to reply to you, to schedule a consultation, and to keep a record of the inquiry so nobody has to ask you the same question twice. We use the technical and analytics records to keep the site running and to improve it.
We do not sell your information, and we do not trade or rent it to anyone for their own marketing.
Who else sees it
Only the service providers who make the site and the practice work: the company that hosts the website, the systems we use to receive inquiries, schedule appointments and send email, and the practice's own administrative staff. Each of them may only use the information to provide that service to us.
We also disclose information when the law requires it, or where disclosure is necessary to prevent serious harm.
Cookies and tracking
This site sets the cookies WordPress needs to serve pages, and analytics cookies that count visits. You can block or delete cookies in your browser; the site will still work. Where your browser sends a Do Not Track signal, we do not add tracking beyond what is needed to serve and measure the page.
Security, and what not to send
The site is served over an encrypted connection, and access to inquiries is limited to the people who need it.
Please do not use the website form to send clinical detail, a diagnosis, or anything you would not want sitting in an inbox. Tell us enough to route your inquiry; the rest belongs in the consultation.
If you are in crisis, do not use this site. Call or text 988, or call 911.
Your choices
- Ask us what we hold about you, and ask us to correct or delete it.
- Ask us to stop contacting you, at any time, by replying to any email or calling the practice.
- Withdraw an inquiry before it becomes a clinical record.
Children
This website is not directed at children under 13 and we do not knowingly collect information from them. Inquiries about a child are made by a parent or guardian.

If this policy changes, the revised version is posted here. Questions about it go to the practice by email or phone, using the details in the footer of every page.
